Do I need to provide anything for residency even though I proved it the first time I came to PHSC?

Yes. New and returning students are required to prove residency by filling out the residency classification form and providing copies of at least two (2) documents that are more than twelve (12) months old and not expired to the admissions department via email, admissions@phsc.edu , or at a campus nearest you.
